At Holy Trinity Primary School, we are committed to inclusion and to ensuring that all pupils, including those with special educational needs and disabilities, are supported to achieve their best possible outcomes.
We follow a graduated approach to identifying and supporting pupils with SEND, using the Assess, Plan, Do, Review cycle in line with the SEND Code of Practice. Needs are identified as early as possible through ongoing assessment, observations, and close communication with parents and carers.

All pupils benefit from high-quality, inclusive teaching. Where additional support is required, reasonable adjustments and targeted interventions are put in place to meet individual needs. Support is reviewed regularly and adapted as required.
Our universal offer (see links below) outlines the inclusive provision that is in place for all pupils. This includes high-quality teaching, a supportive learning environment, and reasonable adjustments that enable every child to access the curriculum and make progress.
We work closely with parents, pupils, and external professionals to ensure support is well planned and effective. Parents are involved at each stage of the graduated approach and are encouraged to share views and contribute to reviews.

The Holy Trinity SEN Policy is a statutory document that is reviewed annually. This one conforms to the most up to date SEN Code of Practice released by the Government. A link to the SEN Code of Practice can also be found below.
The Holy Trinity SEND Information Report outlines the way in which the school identifies children who need extra support and the way in which the support is delivered through the curriculum. It answers a range of questions that parents may have.
